She was given the title Cik Puan Kalsom on the same day of her marriage she became The Sultanah of Pahang, with the title of Her Royal Highness on 30 September 1992.She and the Sultan have one issue; a son together: His Highness Tengku Fahd Mu’adzam Shah Ibni Sultan Hajji Ahmad Shah Al-Musta’in Billah .
